Dive Medical Information

Scuba Diving the Whitsundays with Tornado Dive.

Before booking a Scuba Dive with Tornado Dive, we need you to be aware of the following information, which has been supplied by PADI. A document containing this information will be provided to you on the day you do your dive and you will need to complete it and sign it prior to completing your dive of the Whitsundays.

The purpose of this medical questionnaire is to find out if you should be examined by your doctor before participating in recreational scuba diving in the Whitsundays. A positive response to a question does not necessarily disqualify you from diving. A positive response means that there is a pre-existing condition that may affect your safety while diving and you must seek the advice of your physician.
